Avoid use of "<b>"/"</b>"

Browse source 
* Use "<label>"/"</label>" for form labels.
* Use "<strong>"/"</strong>" for important text.
* Use "<h4>"/"</h4>" for headings.
* Drop "<b>"/"</b>" everywhere else.
